I called American three times and got different answers, but the website is clear: diaper bags don't count toward your carry-on or personal item allowance. My infant had her own seat so she also got a carry-on and personal item just like an adult. Strollers and car seats are checked free — I gate-checked a travel stroller and only once had an agent question it until I showed the AA policy that baby items are exempt.
We gate-checked a Yoyo stroller that collapses into a backpack — it fits in the overhead bin but 80% of the time they still gate-check it. On smaller planes it's gate-checked every time.
